Model Release Form

In consideration of my engagement as a model, upon the terms here that are stated, I hereby grant permission to the designated photographer below, his/her legal representatives, assigns, heirs, and others acting with or under his/her authority to use, copyright, re-use, publish, and republish, photos of me or in which I may be included in that are unchanged or transformed in character, edited without restriction, used in conjunction with my own or a fictitious name, and/or reproduced in any form of media such as (but not limited to) illustration, promotion, and advertising.

I hereby give up any right I may have to approve the completed product or products of such work.

I hereby discharge this photographer and those working under his/her permission any liability in taking such photos, editing them, and publishing them.
